Annual Health and Medical Records

Boy Scouts of America requires that Health and Medical Records be completed annually. Pack 167 chooses to do this at the beginning of each school year for ease of tracking compliance as a Pack. Completion of records applies to ALL participants (scouts, parents, and siblings)—in basic Scouting activities such as local tours and weekend camping trips less than 72 hours in duration.

You do not need to have anything completed by your physician, but you must submit the Part A: Informed Consent, Release Agreement, and Authorization and Part B: General Information/Health History 2014 version of the forms.

Bring your completed, signed, and dated Health Forms to the Pack 167 office at an upcoming Pack event or Den Meeting. 

Health Forms A, B:

  • Applies to ALL participants—in basic Scouting activities such as local tours and weekend camping trips less than 72 hours in duration.

Health Forms A,B,C:

  • A pre-participation physical is needed for resident campers (summer or winter camps) and events 72 hours or more.
